The Saginaw Community Care HUB – a nationally Certified Pathways Community HUB – is a centralized resource serving at-risk populations having health care needs as well as social services needs. Working with provider partners, the HUB coordinates resources across Saginaw County and neighboring counties to ensure those with the greatest need have access to the services they require most. The HUB serves people in these specific groups:
- Adults with two or more chronic health conditions who are eligible for or enrolled in Medicare and/or Medicaid – served via the Saginaw Pathways to Better Health program
- Pregnant women and families with children from birth to age five – served via the Community Access to Home Visiting program
- Children from birth through age 17 who are enrolled in Medicaid and their families – served via the Children’s Health Access Program (Program launching February 1, 2016)
