The Saginaw Community Care HUB’s trained specialists work to align referred patients and clients with services provided through these programs:

Saginaw Pathways to Better Health
Helping people live healthier lives, and lower the cost of health care. Health care delivery can be complex, especially for patients with multiple chronic illnesses. Saginaw Pathways to Better Health assists individuals in accessing the social services they need and addressing challenges that might affect their health care. Saginaw Pathways works together with health care and social services providers and other resource organizations to align patients with the help they need to better achieve their health goals.

Michigan Home Visiting Initiative
Home Visiting – Making a healthy difference for moms, babies and families. Home visiting works with providers of professional services to help ensure that their patients have access to the services they need so that first-time moms have a healthy pregnancy and that children have the opportunity to grow and develop in a safe and stimulating environment.
