How to Help Patients Access Services

Home Visiting – a healthy difference for moms, babies and families.
Michigan’s Early Childhood Home Visiting programs provide voluntary, prevention-focused family support services in the homes of expectant moms and families with children from birth to age five.

The programs connect trained professionals with vulnerable mothers and families to nurture, support, coach, educate and offer encouragement – with the goal that all children grow and develop in a safe and stimulating environment.

The Saginaw Community Care HUB provides Community Access to Home Visiting programs in the Saginaw area.
The HUB will …

  • help clients select the right program
  • make the referral connection
  • follow up to be sure the referral was successful

Home Visiting programs provide support in a number of areas, all in a one-on-one environment to develop positive interactions focused on the importance of maternal health before, during and after pregnancy.

Saginaw Home Visiting programs include:
Nurse Family Partnership – Through ongoing home visits from registered nurses, low-income first-time moms receive the care and support they need to have a healthy pregnancy, provide the right care for their children, and become more economically self-sufficient. These services continue from pregnancy until the child turns two years old.

Healthy Families America – This nationally recognized home visiting program works with families who may have histories of trauma, intimate partner violence, mental health and/or substance abuse issues. Services are offered from pregnancy onward over a three- to five-year period.

Early Head Start Home Visiting – Focused on improving school readiness in young children of low-income families, this comprehensive child development program serves pregnant women, infants and toddlers and their families.

Maternal Infant Health Program – Services include home visitation and care coordination for pregnant women and their infants who are Medicaid-eligible. Services are intended to supplement regular prenatal/infant care by health care providers.

Birth to 5 Program – Open to families living in Saginaw County with children under the age of five, the program includes regular home visits that focus on child development and support utilizing the evidence-based program, Parents as Teachers.
